Roast Log for RP 191/291

I've recently been filling out the roast logs for RP 191/291 certification credit and the page where that is submitted seems to have gone away. The links on http://roastersguild.org/?p=certification and http://roastersguild.org/?p=certification&sub=certLvl all point to https://specialtycoffee.wufoo.com/forms ... sion-form/ which has been returning a 404 error for at least the past couple days. The links worked less than a week ago.

Is there a current location to send these files?
